Copy-trading test suite (deterministic + live)

Full reproducible copy-trading test suite สอง layers:

Single-use refresh tokens (สำคัญ)

cTrader refresh tokens เป็น single-use — แต่ละ refresh returns new refresh token, invalidates old Live fixture refreshes on start, persists rotated token ไปยัง secrets/openapi-tokens.local.json ผลที่ตามมา:
